Qualified Social Worker – Full-Time Locum

Hours: Full-time, Monday–Friday

Rate: Competitive hourly rate, dependent on experience

Total Assist Recruitment are currently seeking an experienced Qualified Social Worker to join the multidisciplinary team at a specialist private healthcare service in Devon.

The service provides rehabilitation and ongoing support to adults with complex mental health needs, with a focus on recovery, rehabilitation and developing independence.

The Role

As a Qualified Social Worker, you will work as part of a multidisciplinary team, supporting individuals throughout their rehabilitation journey.

Your responsibilities will include:

• Managing a caseload and completing social care assessments
• Supporting care planning, discharge planning and transition into the community
• Completing risk assessments and safeguarding work
• Liaising with local authorities, community teams and other external agencies
• Attending MDT meetings and contributing to multidisciplinary care planning
• Supporting individuals with housing, benefits, advocacy and access to community services
• Maintaining accurate and timely clinical and social care records
• Working collaboratively with patients, families and the wider professional team

Requirements

• Qualified Social Worker with a recognised Social Work qualification
• Current registration with Social Work England
• Previous experience working with adults
• Experience within mental health, rehabilitation or inpatient settings is desirable
• Strong understanding of safeguarding, risk management and social care legislation
• Excellent communication and report-writing skills
• Ability to work confidently as part of a multidisciplinary team
• Enhanced DBS or willingness to obtain one
