Are you a qualified Social Worker with experience supporting adults with mental health needs?

We are currently looking for a compassionate and experienced Mental Health Social Worker to join a dedicated team and support individuals to achieve greater independence, stability and wellbeing.

The Role

As a Mental Health Social Worker, you will work with adults experiencing a range of mental health needs, providing person-centred support and helping individuals access the appropriate services and care.

Your responsibilities will include:

• Completing assessments and reviews of individuals' care and support needs.
• Developing person-centred care and support plans.
• Supporting individuals to achieve positive outcomes and maintain their independence.
• Working with individuals, families and carers to understand their needs and identify appropriate support.
• Managing a varied and complex caseload.
• Working closely with mental health professionals and multidisciplinary teams.
• Liaising with health services, housing providers, voluntary organisations and other agencies.
• Identifying and responding appropriately to safeguarding concerns.
• Supporting individuals through transitions and changes in their care.
• Maintaining accurate and up-to-date records and case notes.
• Working in accordance with relevant legislation, policies and safeguarding procedures.

About You

We are looking for a Social Worker who:

• Holds a recognised Social Work qualification.
• Is registered with Social Work England.
• Has experience working with adults experiencing mental health difficulties.
• Has experience completing assessments, reviews and care planning.
• Has a good understanding of safeguarding and relevant social care legislation.
• Is confident managing a complex caseload.
• Has excellent communication and interpersonal skills.
• Can work effectively as part of a multidisciplinary team.
• Is compassionate, resilient and committed to supporting people to achieve positive outcomes.
