Job Description

We are currently recruiting an experienced and motivated Hospital Social Worker to join a busy Adult Social Care team within a hospital setting.

This is an excellent opportunity for a qualified social worker with experience in hospital discharge, assessment and care planning to support patients in achieving safe and timely discharges from hospital.

You will work closely with patients, families, carers and multidisciplinary professionals to assess care and support needs, coordinate appropriate services and ensure individuals receive the right support following discharge.

Key Responsibilities
  • Complete assessments of patients’ care and support needs to facilitate safe and timely hospital discharge.
  • Develop person-centred discharge plans that promote independence, wellbeing and recovery.
  • Work collaboratively with hospital teams, including nurses, doctors, occupational therapists, physiotherapists and community social care teams.
  • Assess patients under the Care Act 2014 and identify eligible care and support needs.
  • Arrange appropriate care packages, placements, rehabilitation support and community services where required.
  • Support patients and their families through difficult decisions regarding ongoing care and accommodation.
  • Identify and respond to safeguarding concerns, ensuring vulnerable adults are protected from abuse and neglect.
  • Undertake mental capacity assessments where appropriate and contribute to best-interest decisions.
  • Liaise with care providers, families and external agencies to coordinate effective discharge arrangements.
  • Maintain accurate case records and complete assessments and reports in line with statutory requirements and local authority procedures.
  • Manage a varied caseload within a fast-paced hospital environment, prioritising urgent cases effectively.
Requirements
  • A recognised Social Work qualification.
  • Current registration with Social Work England.
  • Previous experience in Adult Social Care, ideally within a hospital or discharge team.
  • Knowledge of the Care Act 2014, Mental Capacity Act 2005 and adult safeguarding procedures.
  • Experience completing assessments, support planning and multi-agency working.
  • Strong communication, organisational and report-writing skills.
  • The ability to work effectively in a busy, multidisciplinary environment.
