Pro Social Partners are working in partnership with Medway Council to recruit a Senior Social Worker into their Transitions Team. This is a permanent role in one of the most rewarding areas of practice.

About the team and the role

The Transitions Team walks alongside young people aged 14 to 25 as they move from childrens services into adult social care, which is a big moment in their lives. Day to day you will:

• Hold a caseload of young people preparing for adulthood
• Carry out assessments under the Care Act 2014 and the Children Act 1989
• Plan transitions around education, health, housing and independence
• Work across childrens and adults teams and with health partners

• Registered with Social Work England
• A recognised social work qualification
• Experience with young people with disabilities or complex needs
