Eden Brown Synergy are currently looking for an experienced Social Worker to join the Continuing Healthcare Team in North Hampshire.

Duties and Responsibilities

• Support adults undergoing Continuing Healthcare (CHC) assessments to determine their eligibility for NHS-funded care.
• Complete social care assessments and contribute to multidisciplinary CHC assessments and reviews.
• Gather and analyse relevant information to ensure individuals care and support needs are accurately identified.
• Work collaboratively with NHS colleagues, health professionals, social care teams and other partner agencies.
• Provide advice and guidance to individuals, families and carers throughout the CHC assessment process.
• Ensure assessments and recommendations are person-centred and reflect the individuals needs, wishes and desired outcomes.
• Maintain accurate case records, reports and documentation in line with statutory and organisational requirements.
• Support effective care planning and ensure appropriate services are identified to meet assessed needs.

Pay Rate: £38.00 per hour Umbrella.

Essential Requirements

• Recognised Social Work qualification.
• Current Social Work England registration.
• Previous experience within adult social care.
• Essential knowledge and experience of Continuing Healthcare (CHC) processes and assessments.
• Understanding of the Care Act 2014 and relevant adult social care legislation.
• Strong assessment, analytical and report-writing skills.
• Ability to work effectively with multidisciplinary teams and external partner agencies.
• Excellent communication and organisational skills.
