  • Work collaboratively with families, carers, community mental health teams, health professionals and other partner agencies.
  • Support individuals experiencing changes in their circumstances, including hospital discharge, transitions and changes to care arrangements where required.
  • Apply relevant legislation, including the Care Act 2014 and Mental Capacity Act 2005, in day-to-day practice.
  • Maintain accurate case records and prepare reports in accordance with local authority policies and procedures.
  • Participate in multi-disciplinary meetings to coordinate effective care and support.
Requirements
  • A recognised Social Work qualification.

  • Previous experience within Adult Social Care, ideally working with adults with learning disabilities and/or mental health needs.
  • Knowledge of the Care Act 2014, Mental Capacity Act 2005 and adult safeguarding procedures.
  • Strong assessment, communication and report-writing skills.
  • The ability to manage a caseload effectively and work collaboratively with multi-agency professionals.
  • A commitment to safeguarding, promoting independence and achieving positive outcomes for adults.
