• Deliver high-quality community nursing care to patients in their homes and community settings
• Carry out assessments, care planning and clinical interventions as a Band 5 nurse
• Manage and prioritise a caseload of community patients
• Administer medications and carry out clinical procedures in line with NMC standards
• Work collaboratively with the wider neighbourhood MDT including GPs, social care and therapy teams
• Complete accurate and timely clinical documentation
• Escalate concerns regarding patient deterioration appropriately
• Contribute to the safe and effective running of the community nursing service

Essential:

• Registered Nurse with active NMC pin
• Experience in community or district nursing (desirable but not essential)
• Ability to manage own caseload and work autonomously
• Full UK driving licence and access to a vehicle
• Good communication and documentation skills
• Enhanced DBS check required
