We are recruiting a Prescribing Pharmacist for a fully remote role supporting an innovative Hospital-at-Home service. This service enables patients to safely receive care and monitoring in their own homes through digital healthcare solutions, improving outcomes while reducing hospital admissions.

About the Role

As a Remote Prescribing Pharmacist, you will conduct telephone-based medication reviews with patients enrolled in the service. You will focus on optimising medication regimens, identifying issues early, and making prescribing decisions where clinically appropriate.

This is a temporary, ongoing position, with 40 hours available per week.

Key Responsibilities

  • Conducting structured medication reviews with patients over the phone
  • Reviewing current medications for safety, effectiveness, and adherence
  • Calling patients to discuss symptoms, side effects, and treatment outcomes
  • Amending, initiating, or stopping medications in line with clinical guidelines
  • Clearing and managing routine medication-related alerts
  • Identifying red flags and escalating concerns to nurses or clinicians where required
  • Documenting all consultations and changes accurately on clinical systems
  • Providing clear advice, reassurance, and patient education

What We’re Looking For

  • Registered Pharmacist with Independent Prescribing qualification
  • Experience conducting medication reviews (primary care, community, or remote settings)
  • Strong clinical decision-making and risk assessment skills
  • Confident telephone consultation manner
  • Ability to work autonomously within a remote clinical environment
  • Excellent documentation, organisation, and communication skills
  • Comfortable using digital systems following training
